My Story
The service unit, subsidised by the Hospital Authority, was established in 2023. It extends our elderly discharge service from Hong Kong East to Hong Kong West to Queen Mary Hospital. Through a multidisciplinary team (comprising social workers, nurses, physiotherapists, occupational therapists, speech therapists and care workers), the unit provides appropriate, caring and professional home-based, rehabilitation and community support services for discharged elderly persons. This assistance aims to facilitate post-discharge emergency support, adaptation, rehabilitation and caregiver support to reduce the risk of readmission.
